ACO Participation Waiver

Rocky Mountain Accountable Health Network, Inc. (RMAHN) has entered into the following arrangements related to its participation as an accountable care organization (ACO) in the Medicare Shared Savings Program and that it intends to qualify for the applicable Program Waivers:

A services agreement effective 2/09/2023 between RMAHN and Castell, LLC to provide a range of support services to the ACO and its providers for the benefit of its Medicare patients. The services provided by Castell include patient engagement for scheduling, arranging for preventive care, care coordination with providers, and support with clear and accurate medical record documentation.
